Celebrating the Life of Blaise Alessandro Carrillo

Martinsville, IN — Blaise Alessandro Carrillo, a vibrant, courageous, and deeply loved 16-year-old, passed away on May 26, 2026, in Martinsville, Indiana, following a fierce, yearlong battle with leukemia. Though his fight was complex and filled with difficult complications, Blaise faced every single day with a bravery that inspired everyone around him.
Born on October 29, 2009, in Terre Haute, Indiana, Blaise brought an immediate warmth and energy into the lives of his parents, Sara (Blystone) Brake and Osni Carrillo Sanchez. He grew into an extraordinary young man and was currently a sophomore at Martinsville High School.
To know Blaise was to know pure joy in motion. He possessed a deep, unyielding love for music, and no matter how sick he was, or how difficult the day, you could always find him singing and dancing. He had a profound passion for Broadway musicals, with Cabaret holding a particularly special place in his heart. Blaise had an infectious laugh that could light up the darkest rooms, and a smile so beautiful it etched itself into the memories of everyone he met. He truly left an indelible mark on this world, which now feels undeniably less colorful without him in it.
Family and Legacy
Blaise’s memory will be forever cherished and kept alive by his loving family:
Parents: His mother, Sara Brake (John), and his father, Osni Carrillo Sanchez (Gera).
Siblings: Ava DeLeon, Isaac Rogers, Marina Vazquez, and Camila Vazquez.
Grandparents: Scherelda “Mike” Blystone, William Blystone Sr., and Sonia Sanchez Samayoa and Romeo Carrillo Barrios
Extended Family: A host of aunts, uncles, cousins, and friends who loved him dearly.

Blaise lived his life with that exact theatrical spark, teaching us all to dance through the shadows and sing through the pain. He will be missed beyond measure.
Funeral services will be held at Fitzpatrick Funeral Home, 220 North 3rd St., West Terre Haute, IN on Friday, May 29, 2026 at 4:00 PM. Visitation is scheduled on Friday from 2:00 – 4:00 PM in the funeral home. In lieu of flowers, the family requests memorial contributions be made to Riley Children’s Hospital, 705 Riley Hospital Drive, Indianapolis, IN 46202, in memory of Blaise.
